The following restaurant inspections were conducted by the Lenoir County Health Department:
Annie Bee's Country Cooking, LLC

Observations:
Raw chicken gizzards stored on top shelf above raw pork chops, raw fish, and smoked turkey wings. Foods in coolers shall be stored according to final internal cooking temperatures; Ready-to- eat foods shall be stored on the highest possible shelf; raw seafood, raw pork, raw beef, and raw poultry shall be stored on the lowest possible shelf.
Ice machine contained mildew today.
The following ready-to-eat, TCS foods were not dated in the reach in cooler today: cooked neck bones in grease, cooked pig feet, opened package of hot dogs, opened package of cheddar cheese, opened gallon of milk, cooked chitterlings. Cooked yams were stored in freezer with no date. All ready to eat, TCS foods shall be dated with date of initial prep, cooking, or date of discard.
Label all sauces that are near the front counter area (vinegar, assorted BBQ sauces, etc. )
Spoon used to scoop hush puppies and corn bread stored in water today. All in use utensils shall be stored in water at least 135 degrees F; on a clean, sanitized surface; or in a dipper well with continuously running water.
Clean all dry goods containers that have accumulated build up on lids and handles. Clean inside of reach in cooler and reach in freezer to remove raw meat drippings and food debris. Clean container holding spices on dry storage area.

Observations:
Hand-washing sink at sushi bar area was blocked by sushi rice today. All hand-washing sinks shall be accessible at all times for hand-washing.
The following foods were stored in 2 prep coolers on the main cook/prep line: cut lettuce 71 degrees F; salad mix 74 degrees F; sliced tomatoes 72 degrees F; pico de gallo 72 degrees F; bulgolgi wraps 49 degrees F; Japanese egg rolls 43 degrees F; cooked chicken breasts ( 5 packages) 54 degrees F; cooked chicken ("crack chicken") (2 packages) 51-53 degrees F. All cold holding TCS foods shall be held at 41 degrees F or below.
Bags of cooked chicken (2) found in walk in cooler not dated. Bags of cooked chicken breasts (5) found in 2 front prep coolers not dated. Pico de gallo found in front prep cooler not dated. Bag of cooked chicken (crack chicken) found in front prep cooler not dated. All ready to eat, TCS foods, shall be dated when first prepped or cooked or date of discard.
Two chemical bottles found not labeled today. Employee stated chemical was Fabuloso. All chemicals out of their original containers shall be labeled.
Label all sauces and dry goods containers.
One pan of food sitting on the floor of the walk in cooler. All foods shall be stored at least 6 inches off the floor to prevent contamination.
Bowl found in fried rice today. Paper bowls and cups found in other various dry goods containers. All in-use utensils shall have handles to prevent contamination of the food product.
Three soiled linens found stuffed on shelving on main prep line with seasonings, clean equipment, and other items.
Coolers on front prep line were not maintaining at 41 degrees F or below today. Breaker had popped, causing one cooler to have an ambient air temperature of 73 degrees F. The other cooler on the prep line was maintaining at 50 degrees F. All coolers shall maintain a temperature of 41 degrees F or below. Coolers were being serviced during inspection. Do not use coolers until they are maintaining 41 degrees F or below. Repair broken track on cooler drawer under grill. Gaskets are torn on prep cooler on front line by hand-washing sink. Gasket is torn on reach in cooler across from 3 compartment sink. Glass door freezer doors are popping open when one is closed. Large amounts of ice was present at the bottom of this unit. Repair and/or defrost. Replace broken/missing lid on soup kettle.
White prep board on prep cooler by hand-washing sink contains heavy scoring.
Prep cooler contained accumulated food debris and single service that had been dropped behind the lid. Glass door freezer needs to be defrosted and cleaned. Clean dry good containers.
Hand-washing sink beside prep cooler is very slow to drain.
Kings Restaurant

Observed several pans and deli slicer with food remains stuck on them. Observed several containers with left over stickers still on them. Inside of ice machine visibly soiled.
Fried chicken on bar and ribs in the prep area were not temping 135.
Observed live roaches in kitchen and flies throughout.
Ice sitting uncovered in banquet room.
Observed several employees with beards, wearing no guards.
Styrofoam cup being used for scoop in bbq . Approved scoop with handle must be used to prevent contamination from occurring.
On hallway, all dishes stacked and still very wet. Stainless steel pans and salad bowls stacked wet.
Rusted shelves in 2 door continental unit. Equipment missing front doors and plates/coverings, so grease is building up inside equipment.
Rusty Shelving units, equipment, outside/inside refrigerators, floor of walk in coolers, wire racks, clean dishes table, plate rack, dish racks, plate carts, etc. Establishment shall clean.
Leak present at hand-wash sink in chop room.
Ceiling tiles have damage, and holes present on ceiling in storage room. Wallboard coming off the wall and loose in the WIC area.
Floors, walls and ceilings need additional cleaning due to build up of dust, grease, flour, etc. Backsplash at plate prep counter is heavily soiled. Bacterial growth still present on ceiling in storage room.
Along fryer line, lighting measured from 16 to 20 foot candles, with above the fryers being 10 to 15 foot candles. Required lighting intensity at work surfaces shall be 50 foot candles.
Areas designated for employees to eat and drink, shall be located so that food, equipment, linens, single service/use items are protected from contamination. Employee food stored in chest freezer in packing room.

Raw bacon stored directly on top of hot dogs in reach in cooler. Container of shell eggs stored on top shelf above produce, such as tomatoes, cilantro, peppers, etc. Foods shall be stored according to final internal cooking temperatures in coolers.
Onions & tomatoes 57 degrees F; Salsa verde 86 degrees F; Salsa rojo 92 degrees F; shredded lettuce 62 degrees F. All cold holding TCS foods shall be held at 41 degrees F or below.
Hot dogs dated 1/7 in reach in cooler. Shredded cooked chicken did not have a date. Opened gallon of milk was not dated. All Ready to eat, TCS foods shall be dated when initial ingredient is prepped or cooked and/or discard date.
Utensils stored food contact surface up. All utensils shall be stored food contact surface down to prevent contamination.
Wooden cutting board used to cut produce, such as cilantro, is in poor repair. Cutting boards that have scoring that breaks that surface can no longer be easily cleaned.
Provide a covered waste bin in female restrooms. Need covered trash cans in each stall or main area of the restroom. Each stall and main restroom area had trash cans, but no lids.
Paint is peeling on floor in multiple areas. Floors, walls, and ceilings shall be in good repair.

Label all shakers, sauce bottles, and jars with their contents. These items contained sauces, seasonings, and oil and vinegar, etc.
Paper cup being used to scoop sugar. All in use utensils shall have a handle to prevent contamination of food. All dry good shall have scoops with handles, with handles sticking out of the food.
Cleaning needed on all bread racks; accumulated food debris was present. Clean shelving where extra drinks were stored.

Facility did not have written procedures to follow for clean up when responding to vomiting or diarrheal events.
Metal corners around Hot holding area for nuggets and fries are very sharp and in poor repair. This may be a potential hazard for employees.
Clean inside of prep cooler (bottom coolers) on main prep line. Clean inside of cooler under area where raw hamburgers are stored for cold holding. Clean inside oven (for cooking potatoes) and area under flat top grill to remove accumulated food debris.
Provide a covered waste bin in female restrooms. Restrooms (stalls or main restroom area), shall have trash cans with lids. One stall only had a trash can with lid. Trash can in main restroom area did not have a lid. The other 2 stalls did not have trash cans. Covered waste bins shall be provided in all stalls and/or in the main restroom area.
Areas of floor in poor repair throughout the facility. Floor tiles are broken by drain at 3 compartment sink and under main prep line. Carpet is in poor repair in dining room area; areas are carpet are peeling up. Remove cardboard behind fryers.
Provide required lighting intensity. Replace burned out bulb in reach in freezer. Reach in coolers shall have at least 20 ft candles of lighting.
Chef & The Farmer

Observations:
Employee observed coming into work and immediately turned on equipment, without any hand-washing occurring.
Several utensils, plates and bowls observed to have food residue or left over date marking stickers on them.
Deli slicer observed to be soiled with left over cheese.
Many items in WIC above 41 degrees, due to being stored next to a large container of hot soup that was put into the fridge to cool. Items that were out of temperature: brussell sprouts (47), black beans (47), cut potato fries (47), sweet potato (43/44), large container of tomato soup (56), small container of tomato soup (47), pickled squash (44/45).
On front line, medium container of roots and container of pickled onions and cucumbers, container of cooked grilled chicken missing date mark. Pecan relish only had a frozen date mark on it. Several containers of the same item had several different dates listed.
Clean Utensils stored in soiled containers in the kitchen area.
Drawer missing from lift-top cooler 1 on front line. Large leak present behind front cook line, potentially an issue with the hood vents as well.
Rust present in four drawer unit (in front of hand-wash sink for front line). Observed several lids cracked. Water on the floor inside of the unit (tucked in the corner on the front line (lift-top unit with 2 doors and 2 drawers).
Build up of crumbs, debris, etc observed at the bottom of the refrigerators and cooling drawers, underside of corner lift-top on front line, on the racks and gaskets on refrigeration equipment. Mold present on drawer tracks on unit in front hand-wash on front line.
Large amount of grease built up behind equipment on front line. Crumbs, debris, etc. present on the floor throughout the kitchen under equipment and shelving. Vents on refrigeration units have dust build up present. Mold build up is present on the pipes under the hand-wash sink on the front line. Much additional cleaning is needed.
Discard items and litter stored on the premises as they are not necessary to operation and maintenance. Discussed relocated unused dishes with person in charge to storage to avoid build up from occurring.

Sugar container was not labeled today. Sauce bottles, containers with dry ingredients, and assorted drinks made in-house not labeled today. Horchata contains milk, which is a major allergen. All drinks made in-house shall be kept labeled.
Bowls for salsa, guacamole, and other food items were stored food contact area up. All clean equipment and utensils shall be stored in a self draining position that allows for air drying and covered or inverted.
