Lenoir County restaurant inspections

The following restaurant inspections were conducted by the Lenoir County Health Department:

Hibachi Express 

2405-C N Herritage Street, Kinston 

Date: 1/13

Score: 95

Observations: 

Person in charge is not a certified food protection manager. 

Bottle of green chemical and bottle of blue chemical were not labeled today.

Several containers and bottles of ingredients, seasonings, and sauces were not labeled today. All containers that container foods, such as seasonings, spices, water, oils, sauces, etc. shall be labeled. 

In-use utensils at hot holding area (big spoon for rice and tongs) were being stored in water. Water in containers were 125 degrees F and 127 degrees F. If in-use utensils are stored in water, the water shall be maintained at a temperature of 135 degrees F or above.

Single service forks at spoons at front counter area were stored in different directions for pick up by PIC. All single service and single use articles and cleaned and sanitized utensils shall be handled, displayed, and dispenses, so that the contamination of the food contact area is prevented. 

Handle and outside areas of zucchini chopper are rusting and chipping. 

Lid on dumpster is broken. 


Hardee’s

Observations: 

Cooler at main prep line is not maintaining at 41 degrees F or below. Hinge on one door does not make a complete close.

Sifter was torn on the sides, in biscuit prep area, allow for wires to potentially break off into food causing a physical hazard.

Clean inside of reach in freezer to remove accumulated food debris. Detail cleaning needed around biscuit prep area. Clean broiler/grill area.

Floor cleaning needed under front line and main prep line under all equipment and counter areas. Visible debris and build up seen in these areas. Floor cleaning needed in walk in cooler. Clean ceiling around vents and on ceiling tiles were build up of dust in present. 

Floor tiles by mop wash area are missing grout. Floors tiles are also in poor repair in this area.

Lighting over biscuit prep area is 26-32 ft candles. Lighting over grill area, fryers, and prep line is 22-44 ft candles. Lighting shall be 50 ft candles in all food prep areas.


Kentucky Fried Chicken

Observations: 

Clean ledges where sauce bottles are stored on main prep line. Clean area where colored tags are located to remove accumulated food debris. Clean inside of hot holding units (top shelf prep area) to remove food debris and grease.

Garbage dumpster is missing a drain plug. Drain plugs shall be in place in all dumpsters.

Door open on cardboard dumpster today. Keep all lids and doors closed on dumpsters to prevent pest or rodent presence.

Clean floor in storage room where boxes of soda are stored. A spill has occurred in this area and syrup is present on floor tiles and on grout in between floor tiles.

Wall is rusting at 3 compartment sink area beside dishwashing sink drainboard. Floor is in poor repair in this area. Wall of chicken walk in cooler beside door is in poor repair. Paint is peeling on walls at dishwashing area entrance, wall outside of chicken walk in cooler, and under soap dispenser at handwashing sink by dishwashing area. 

Arby's 

4173 W Vernon Avenue, Kinston

Observations: 

Ice machine contained black mildew around ice shield/baffle area. 

Label containers and shakers holding cinnamon and other food products out of their original container.

Tongs for pastries stored on hook with keys at front counter area. Store all in use utensils in areas to prevent contamination.

Replace cutting boards at sandwich prep area that are heavily scratched. Bacteria may harbor in these areas. Boards shall be smooth and easily cleanable with no deep scratching. 

Replace, remove, or repair equipment such as glass cooler that is not working. Repair loose handle on top of milkshake machine. Repair hot water faucet (nozzle) on 3 compartment sink; it is very loose and continues to spin on faucet. Remove dish machine if not in use.

Clean inside of reach in freezer to remove food debris. Clean shelving in walk in freezer to remove ice build up. Clean area at front counter where tea urn is stored and tongs for pastries. 

Faucets at handwashing sink in ladies room are not providing adequate pressure for handwashing. There is very little water coming out of the faucet to wash and rinse hands properly. Repair or replace faucet so that there is an adequate amount of water pressure provided to properly wash hands.

Floor tiles throughout the facility are missing grout, cracked, and/or broken. Replace/repair broken or cracked floor tiles and replace grout where missing.

Clean floor under milkshake machine and under other equipment on main cook line to remove accumulated food debris. 

Need more lighting over handwashing sink by 3 compartment sink; lighting in this area is 7-9 ft candles. Need at least 20 ft candles of lighting at handwashing sinks. Need more lighting at milkshake machine and beverage area. Lighting in these areas are 28-36 ft candles. Need at least 50 ft candles of lighting in food prep areas. Lighting in restrooms are 9-15 ft candles. Need at least 20 ft candles of lighting in restrooms.

Handy Mart

2325 Hwy 258 N, Kinston

Observations: 

Provide at least 100F water at handsinks. The handwash lavatory at the prep line has 71 degree F hot water. the one at the self serve is not 100. the Barhroom lavatories are erratic ranging from 83-120. Maintain at least 100 degree hot water at all lavatories.

The sanitizer at the 3 comp sink was not at least 200 ppm. Manager remade the solution to correct strength. 

The counter mount can opener is visibly soiled.

Ham, chicken, sausage in the display case were not being held at 135 degrees F or greater. Alll hot foods shall be held at least 135 degrees F.

No accurate thermometer was located. 

Both prep unit cutting boards are heavily scored and not easy to clean and sanitize. These shall be replaced.

Cutting or piercing parts of can openers shall be readily removable for cleaning and for replacement. The counter mount canopener is rusted and soiled. It shall be replaced with the ANSI approved kind that can be taken apart easily for regular cleaning.

The gasket on the walk in cooler door is torn loose.

All exposed piping at sinks observed to have heavy grease/dust buildup. Sides of fryer, hood, filters, black plastic rolling cart, equipment handles, 2 comp sink and pretty much all other equipment shall be cleaned from observed grease/dust buildup and dead bugs from 2 comp sink. 

Keep dumpster, outside waste containers covered with tight-fitting lids or doors. Dumpster door was open.

Major cleaning needed on the floors under ALL refriferators and other equipment. The walls observed to need better cleaning. Floors in the walk in cooler need more cleaning. The ceiling vents are rusted, greasy, and moldy. These shall be cleaned or replaced.

Royal Farms 

4206 W Vernon Avenue, Kinston

Observations: 

Two out of three handwashing sinks did not have paper towels for drying hands initially during inspection today. 

All handwashing sinks shall have NC DHHS handwashing signs posted.

Lids and containers used for storing potato wedges after cutting found dirty today; contained many areas where old stickers/labels were present. Areas on these containers were sticky from sticker/label residue. Clean containers and pans found with old stickers and labels. Stainless steel pans contained grease build up. Prep/cutting board at front line cooler contained old dried food debris at areas where board is attached to stainless surface. Cutting board shall be removed; washed, rinsed and sanitized. Stickers/labels on pans and containers shall be removed when washing. Areas where stickers/labels are present shall be cleaned to where surface is smooth, nonabsorbent, and clean to sight and touch.

Sausage/egg/cheese waffle sandwiches in hot holding grab & go customer self serve area were 116-118 today. All hot holding TCS foods shall be held at 135 degrees F or above.

Employees seen handling opened single service, food, clean equipment and utensils with long, artificial nails. Long nails may puncture gloves. All employee handling food, clean equipment and utensils, and single service shall not wear artificial nails and nails shall be clean and short.

Tongs used to get chicken out of hot holding cabinet were hanging on the handles of equipment today.

Containers holding clean equipment, pieces to equipment, and clean utensils found dirty today with accumulated food debris.

Clean pans and other containers holding single service items such as bags for food items. Employees are standing on bottom shelf at front counter area (shoe prints were seen along the front counter shelving) where single service items are being stored. Tops of Plastic aprons laying on the floor of facility. Single service items and articles shall be stored in areas and in a manner to prevent contamination.

Counter/handwashing sink in women's restroom is in poor repair. Sink is coming off of vanity area, leaving a gap where sink may eventually fall. 

Much cleaning needed on nonfood contact surfaces throughout facility today. Large amounts of old food debris seen accumulated in small prep cooler on front line. Cleaning needed around hinges and white board at hot holding unit; board shall be removed from this area and washed, rinsed, and sanitized. Accumulated food debris seen in cracks and crevices of this area. Clean inside of hot holding cabinet to remove old food debris. Clean inside of 4 door cooler. Detailed cleaning needed throughout facility. 

No covered waste bins are in the main section of women's restroom or inside the stalls. Covered waste bins shall be provided either inside the women's stalls or in the main section of the restroom for feminine hygiene products. Bins are provided in stalls, but they are not covered. Trash can is also not covered in main section of the restroom.

Men's toilet was dirty today. Hand-washing sinks in restrooms and prep/cooking area of the establishment need to be cleaned.

Garbage dumpster does not have a drain plug. Lids are broken on garbage dumpster and are laying on the concrete pad behind dumpster today. All waste receptacles, including outside dumpsters, shall be designed and constructed to have tight fitting lids, doors, or covers. Dumpster needs to be replaced.

Build up of flour, grease, food, dirt, etc around and under/behind equipment due to continuous use. Establishment shall clean frequently to prevent build up.

Mops laying flat on floor of facility today by mop wash area. Mops shall be placed in a position that allows them to air dry without soiling walls, equipment, or supplies.

Two bulbs burned out in hood above fryers. Lighting shall be at least 50 ft candles in food prep areas. Replace or repair fixtures such that adequate amount of lighting is met.
