The following restaurant inspections were conducted by the Lenoir County Health Department:

R-Mart Inc., DBA The Farmhouse

Observations: 

Employees observed not wearing hair restraints. 

Equipment handles, doors, and gaskets have slight build up present. 

Tokyo House

1201 W Vernon Avenue, Kinston

Observations: 

Several raw meats that have been removed from original packaging is stored above and beside ready-to-eat foods in freezer. 

A few tongs and bowls and the food preparation sink have not been cleaned thoroughly. 

Several containers of dry ingredients are not labeled. 

Several foods stored in direct contact with t-shirt/grocery store type bags. 

Several wet wiping cloths were on sushi board, counter top, cutting board, etc. 

Bowls used for scooping dry ingredients are not approved due to no handle. 

Fancy single-service trays up front within customer access are stacked and not covered or inverted. 

Rusty door for left dorm refrigerator, peeling finish on white table top, several rusty shelving units, hard-to-clean wooden areas up front, damaged and rusty lid on large chest freezer in back with insulation exposed, some type of metal hose wash compartment of 3-comp sink is extremely rusty. 

There is rusty metal support to hold the rack of clean drinking glasses on top of the rusty 3-comp sink compartment, rusty squeegee, soiled equipment handles, homemade cushions for sharp edges on the top of the homemade white table are not cleanable, cement block used for supports are rough, wooden plumbing pipe support is bare and absorbent, etc. 

Two dorm-type refrigerators are not approved for commercial use. Glass-door refrigerator is not approved for the storage of food that is not packaged. 

Equipment, sinks, bin for the storage of the bag of tempura batter, handles, legs, automotive plastic used to line shelving in cook area, etc. are soiled. 

A substance was splattered on stainless steel wall behind grill. 

Wet mop stored on the floor with head down.

Observations: 

Several utensils have not been cleaned thoroughly. 

Small container of chicken salad in top of lift-top refrigerator is 43-44F. Should be 41F or below. 

No consumer advisory for undercooked hamburgers. 

Dry ingredients for french fry seasoning and a container of salt are not labelled. 

Few wiping cloths laying on prep table and sink. 

Few stacks of single-service pans in storage room are not covered and not inverted. 

Most shelving in walk-ins are rusty. 

Fryers and fryer baskets have build-up. 

Wire shelving in walk-in has black crud. 

Gasket and floor of lift-top is soiled. 

Paint on floors in kitchen and in restrooms is worn/wearing. 

Corner where wooden baseboard meets in men's restroom is "damaged". 

Floor in kitchen is black due to lack of cleaning/difficulty cleaning. Walls in can wash near the bottom are heavily molded and black. 

Lighting in storage room is very dim, should be 10 foot candles or more.

Observations: 

Open package of hot dogs were not date marked. 

Two stem-type thermometers are not accurate, one is high and one is low. 

A few wet wiping cloths laying on sink, prep table, etc. not being used, should be kept in sanitizer solution. 

Employee beverage was being stored on drainboard with clean utensils. 

Ceiling leaking in 2 places but not where food, equipment, or utensils are being contaminated.
