Salary Range: $32,877 - $50,027

Description of work: The Social Worker II is responsible for an assigned patient case load of county residents that varies. Each patient assignment is based upon a referral from a medical provider or other agencies providing services to children 0-5 years who reside in Lenoir County. The social worker is responsible for: 1. assisting with obtaining community resources to ensure positive health outcomes 2. to teach clients and their families the proper use of medical services 3. follow-up with any risk factors that may be detrimental to positive health outcome by using the Life Skills Progression, SWYC, and Community Needs Assessment; developing and implementing a plan of care to ensure healthy outcomes; and providing data information to the North Carolina Division of Medical Assistance's data base – Virtual Health (VH) The Social Worker II is responsible for the quality of the health care delivered to the patients served by the agency. This work requires specialized knowledge in advanced level Social Work assessment utilizing theories and practices in the provision of patient care which include:
1. principles and techniques utilized by advanced level social work
2. coordination of services
3. accurate knowledge of the Case Management for At Risk Children Program 4. working knowledge of clinic services that include referral, CMARC Referral Screening will be entered within 7 days of receipt)

1. Extensive knowledge of family and group dynamics and intervention techniques
2. Extensive knowledge of methods and principles of casework
3. Extensive knowledge of social work principles, techniques, practices, theory and application
4. Knowledge of programs and resources in the community including governmental and private organizations
5. Knowledge of Virtual Health
6. Skills in communication techniques and strategies
7. Skill in counseling techniques which includes interventions for assessment of psychosocial, behavioral and psychological aspects of problems
8. Skill in stress management
9. Ability to work with individuals/groups/community
10. Ability to interpret program goals and objectives to groups with varied educational backgrounds and cultures
11. Ability to express ideas clearly and concisely
12. Ability to plan and execute work effectively
13. Ability to establish and maintain effective working relationships with other disciplines on staff and in civic, legal, medical, social and religious organizations as well as client populations
14. Ability to exercise good social work judgment and make decisions independently
15. Awareness of and inter-relationship with other agencies in the community
16. Expertise in the area of Care Management for At Risk Children Program
17. Ability to speak to individuals or groups on issues relating to the public health areas of current assignment
18. Knowledge of current issues in public health
19. Ability to increase knowledge and keep current of social work changes by attending workshops
20. Knowledge of available resources and organizations and ability to coordinate these as needed
21. Ability to secure patient or family cooperation
22. Ability to record, interpret and utilize records and reports
23. Stay abreast of current social and economic problems relating to public health within the community

Required Minimum Training: The social worker (MSW, BSW, or meet SW II qualifications) must have a minimum of one year of experience in health and human services and have experience working with pregnant women and their families.
