Observations: 

Employees not taking proper steps when washing hands. 

Spinach and cheese quiche held past date mark. 

Chemical cleaner spray bottle hanging from splashguard with nozzle facing takeout trays. 

Walk-in lightly soiled. 

Mop sink has a leak on cold side.

Observed spray bottle with blue liquid with the name rubbed off. 

Observed 4 ft. candles of light in the walk in.

Employee was not wearing a beard net while preparing and handling food. 

Observed wet cloths on the counter and on the cart next to the fryer.

Observed raw pork chops and other raw meat in ziplock bags mixed together and above ready to eat foods in the two door reach in fridge. 

Observed wall behind small ice machine up front coming away from the brick wall behind and leaving gaps in the joints. 

Handy Mart

Observations: 

Flour for breading chicken was not labeled. 

Equipment nonfood contact surfaces needs additional cleaning. 

Dumpster doors open. 

Walls, floors, and ceilings need additional cleaning.

Observations: 

No employee health policy on file. 

No vomit and diarrhea clean up plan on file. 

Employee was observed putting on gloves with out washing hands. 

Observed sausage patty at a temp. of 114, sausage dogs at 116 and cooked eggs at 103 under the heat lamps. 

Two bulbs were not working right above where the product was hot holding. 

Breaders not labeled. 

Observed two lights out under the warmer causing food to be hot held below 135F. 

Observed small holes in board above the prep sinks. 

Observed shielding on heat bulbs under warmer on front counter peeling off of several bulbs.
