Help wanted: Lenoir County Health Department: Public Health Nurse II
Public Health Nurse II: Regular, Full-Time, Position #5110-20-054 Hiring Salary: $44,516.15
The primary purpose of this position is to provide Care Coordination for Children, case management and community outreach services. Home visits are made for the purpose of case management or any other nursing activity that involves functioning independently outside of the agency for seeing children 0-5 years old.
Other responsibilities include:
1. To assist with obtaining community resources to ensure a positive health outcome
2. To teach clients and their families the proper use of medical services
3. Follow up with any risk factors that may be detrimental to a healthy outcome by using the Life Skills Progression, Community Health Assessment and implementing a plan of care
4. To ensure healthy outcomes and provide updated data to the North Carolina Division of Medical Assistance date base. (Virtual Health – VH) Minimum Education and Experience: A qualified candidate must be licensed to practice as a Registered Nurse by the NC Board of Nursing.
Minimum education and experience requirements include graduation from a four-year college or university with a B.S. Degree in Nursing which includes a Public Health Nursing rotation and one year of Public Health Nursing experience; or graduation from an accredited school of professional nursing and two years of professional nursing experience including one year in public health; or an equivalent combination of training and experience.
